Preparing for NDIS plan reviews, accessing the NDIS, assisting with a change of circumstance or to review housing and/or support requirements to acheive individuals goals.
Using a combination of standardised assessments, observation and informal interviews, we make recommendations to assist individuals increase or maintain their independence in the areas of: personal care, domestic activities of daily living, vocational goals, leisure activities, habits, roles and routines.
We work collaboratively to design individual support systems for housing transitions, navigating where individuals want to live, who they want to live with, and implementing formal and informal support networks.
Housing transitions are a time of change and uncertainty. We work collaboratively to help individuals navigate housing and support options available.
Recovery and living with enduring mental illness is an ongoing journey, and Ready OT assists individuals to find the tools and strategies that enable them to participate in the things they want to do, need to do and have to do.
Through activity a therapeutic relationship is built. Whether this is sharing a cup of tea, going for a walk, or listening to music. Utilising our unique assessments and interventions, we equip individuals and their support networks with education, resources, and strategies to maintain their wellness ongoing.
Goals around employment? Education? Career change?
Returning to the workforce or engaging in education can be an overwhelming time. Ready OT works to develop the skills required to return to work/study successfully for the individual.
